Because life can suddenly take an unexpected turn, it's good to know you're covered if the worst occurs. TreeHouse offers you different levels of coverage to meet your needs.

Name a Beneficiary

It's important to name a beneficiary in the event of your death. Designate your beneficiary in Workday. You are automatically the beneficiary for spouse and child coverage.

 

Basic Life and AD&D Insurance

Company pays, coverage is automatic

TreeHouse provides basic life and AD&D insurance to assist you and your family in the event of a death or dismemberment. These benefits are fully paid for by TreeHouse. Coverage is automatic; you do not need to enroll.

Benefit is equal to 1.5 times your annual base salary, up to a maximum of $1,500,000.

 

Voluntary Supplemental Life and AD&D Insurance

You pay, you must enroll

You can also choose to purchase additional life insurance for yourself, your spouse and your child(ren). You pay the full cost of any supplemental life insurance and/or supplemental AD&D insurance coverage. Your specific benefit costs are based on your age and coverage amount elected and will be provided as you enroll through Workday.

Your Voluntary Supplemental Life Insurance Options*

If you elect Supplemental Spouse Life Insurance for your spouse, the coverage amount must be less than the total life coverage amount you have for yourself. (Note: The basic life and supplemental life are added together to determine amount of employee life coverage.)​

  • Supplements the basic coverage provided by TreeHouse
  • Select coverage between 1 – 5 times your annual base salary, up to a maximum of $2,000,000 (includes your company-provided basic life insurance benefit)

Your Spouse**

  • Available in increments of $25,000, $50,000, $75,000 and $100,000

Your  Child(ren)**

  • Available in increments of $5,000, $10,000, $25,000
  • Children can be covered up to age 26

*This coverage is tied to your employment and ends if you leave the company unless coverage is converted to a personal policy.
**You must first elect coverage for yourself before you can elect coverage for your dependents.

Your Voluntary Supplemental AD&D Coverage Options*

  • Supplements the basic AD&D coverage provided by TreeHouse
  • Select coverage between 1 – 5 times your annual base salary (maximum benefit is the lesser of 5 times your annual base salary or $1,500,000)

You and Your Family
(Spouse and/or Child(ren))

The available benefit will differ depending on who you choose to cover and the amount you elect for yourself

Your spouse only 50% of employee's AD&D election
Your child(ren) only 15% of the employee's AD&D election
Your spouse and child(ren) Spouse: 40% of employee's AD&D election

Children: 10% of employee's AD&D election

Evidence of Insurability (EOI)

Voluntary life insurance elections for you or your spouse over a certain amount may require Evidence of Insurability (EOI). The rules vary depending on the type of enrollment.

Guaranteed Issue

When electing supplemental life insurance at your initial enrollment, you can elect up to $750,000 for yourself and up to $25,000 for your spouse with Guaranteed Issue (GI). You will not have to submit Evidence of Insurability (EOI). Increases to supplemental coverage amounts after your initial enrollment are subject to EOI.

New Hires or Newly Eligible:

  • EOI will be required for your own voluntary life insurance election if you elect coverage of more than $750,000.
  • EOI will be required for spouse voluntary life insurance elections of $25,000 or higher.

Life Events After Open Enrollment and Subsequent Open Enrollment periods:

  • EOI will be required for any increase in your own voluntary life insurance.
  • EOI will be required for any increase in spouse voluntary life insurance.

After electing coverage, you will receive more information if you need to take further action to enroll.

What Is AD&D Insurance?

AD&D insurance provides a lump sum benefit in the event of your accidental death or dismemberment. TreeHouse provides you with basic AD&D coverage along with your basic life insurance policy and you have the option to increase your coverage and include your spouse/dependents with voluntary supplemental AD&D insurance.
